A study analyzing over 109,000 incidents shows that while these failures have become more common, recovery times remain stagnant, with companies often reliant on external engineers for fixes. This highlights the critical need for organizations to implement robust monitoring and controls over their AI agents to avoid runaway costs and unintentional damage. Financially, the risks can be staggering, as demonstrated by a case where an AI coding assistant generated nearly $3,800 in costs over four days without anyone noticing. Furthermore, an e-commerce client experienced a tenfold increase in costs, surging from $5,000 to $50,000 monthly due to unoptimized queries and recursive agent loops.
